It doesn't get simpler than this, folks.
On this page you need literally let us know what classes you took during your eighth, ninth and tenth grade years.

DO NOT WRITE YOUR GRADES DOWN ON THIS PAGE!

We'll see your grades on your transcript, but we need this separate page for review purposes. It shouldn't take you more than a couple minutes to complete. Also, don't forget to let us know what school you were attending during those years.

I honestly can't think of much more to say about this part of the application, other than now we're moving onto the parts that are going to require some effort on your part...
